iHome health professionals are specially trained to deliver high-quality care that helps patients recover and regain strength, health, and independence. Skilled Nursing - conducts patient and environmental assessments, coordinates the care provided by all team members and provides treatments as ordered by patient’s physician
Physical Therapy - treats patients who have difficulty walking, transferring from multiple surfaces, history of falls, and/or a decline in function and balance or strength deficits
Occupational Therapy - provides therapy for patients with coordination problems, delayed fine or gross motor skills, decreased body awareness and difficulties in relation to their environment (bumping into furniture), problems with activities of daily living such as dressing, eating, and personal care
Speech Therapy - offers treatment for those with speech/language disorders and instructs on the use of communication assistive devices
Medical Social Worker - assesses eligibility for public assistance, conducts psycho-social assessments, aids in longterm planning, provides counseling and crisis intervention. Connects patients with appropriate community resources
Home Health Aide - offers assistance to patients needing personal care required to maintain the patient’s health or to facilitate treatment of illness or injury. May also help with light cleaning and meal preparation as beneficial to the patient’s welfare
